Help Desk Requests
New User Account [Envision Employees]
Select if you would like to request a new user account. This user account will be used to access Envision IT resources, including RIS, PACS, and Email.
New User Account [Referring Offices]
Select to request new accounts for employees of referring offices.
Change User Access
Select if you would like to modify an existing user account. This user account will be used to access Envision IT resources, including RIS, PACS, and Email.
New Hardware Request
Select if you would like to request new hardware such as a PC, printer, or scanner.
New Software Request
Select this if you would like to request additional software to be installed.
General IT Request
Select this if you have a request for IT that has not been listed above.
Temp Tech Accounts Request
Select this to request accounts for a temporary technologist.
New or Modify Radiologist Request
Select this to request an account for new Radiologist.
File Restore Request
Select this to request file to be restored to a previous date.
Help Desk Issues
Select if you are having issues logging in and would like to have your password reset.
Select this if you are having an issue with an IT device. Examples: PC, Laptop, Printer, Scanner, Card Reader.
Select this if you are having an issue with an application. Examples: email, Microsoft / Libre Office, web browsers, Imagine, Keybank.
Select this if you feel your system has been compromised or if you would like to report any other Information Security matter.
Other IT Issue
Select this if you have any IT issues that are not addressed above.
Ambra: Gateway Installation
Select to request an Ambra gateway installation.
Outside Read Radiologist Assignment
Select to request a radiologist change for an overread/outside read location.
Select this option to make a change to an existing modality or for a new modality install.
Image Share/Outside Read
Select for image sharing requests to or from Envision Radiology, or for outside (overread) read requests.
Select to request a new or change to existing imaging workflow.
Physician Image Routing Request
Select to request image routing for a physician to a PowerShare destination.
Ambra: General Ambra Issues
Select to report an issue with Ambra.
Select to delete or move images.
Select this option for CD/DVD creation issue.
Select to report issues with reading or modality worklist.
Select to report errors, slowness, or functionality.
Select to report a problem with an existing imaging workflow.
Add CPT Request
Select to request a new exam or add existing exam to a center in RIS. *Requires Approval*
CPT Modification Request
Select to request additional access for an employee's RIS account.
Report Bug or Error
Telephones & Faxing
New Telephone Request
Select if you have a telephone request ie. new direct dial phone number or new phone.
General Telephone Issue
Select to report a general issues with telephones or conferencing devices.
Inbound Call Routing
Select if you are having an issue with incoming calls.
Outbound Call Routing
Select if you are experiencing trouble making outbound calls.
New Faxing Request
Inbound Fax Issue
Select if you are experiencing issues receiving faxes.
Outbound Fax Issue
Select if you are experiencing issues sending faxes.
Other Fax Issue
Select for any fax issue not addressed in other forms.
New Networking Request
Select to request a network implementation or enhancement.

General Network Issue
Select if you are experiencing network issues ie. slow transfers, wifi not working.

Select to submit a general EMR inquiry/request.
